Community First Research and Demonstration Waiver


Waiver Application-

The Executive Office of Elder Affairs, Executive Office of Health and Human Services and the Office of Disabilities and Community Services are actively working to implement a vision of Community First. Community First is an overarching policy that enables citizens to be empowered to age with dignity and independence in their communities through access to person-centered integrated systems, supports, and choices. This policy reflects collaborations among individuals, families, private partners, and government and requires a range of mechanisms to support the effort, one of which is described below.

The Community First 1115 waiver application was submitted to the Centers for Medicare and Medicaid Services (CMS) in December 2006. This waiver application reflects input provided by participants at several waiver stakeholder meetings. The Executive Office of Health and Human Services and the Executive Office of Elder Affairs will now work with CMS to formalize details of the Demonstration and, with the continued participation of stakeholders, to prepare for implementation.

The Community First 1115 waiver concept paper reflects concerted interagency efforts to develop a Community First 1115 waiver to improve the long term care system. This concept paper was submitted to the Centers for Medicare and Medicaid Services in June 2006 and reflects suggestions raised by participants at two regional community forums held in December 2005 and January 2006 and other internal and externalstakeholders.

This document contains a summary of the issues, ideas, and suggestions that participants provided to us during and after the community forums that were held in December 2005 and January 2006, as well as a summary of the changes that were made to the concept paper based onstakeholder ideas and suggestions.

 

Community First 1115 Waiver Public Information Session- August 15, 2008
